Had someone a little while back with a similar problem... IIRC, they solved it by mapping D-pad buttons to keyboard keys.  Not sure if it was a built-in function of Windows/Steam, or an extra program running in the background, though.

Mod reinstall - You shouldn't need to.  Just set the 7H and FF7 executables to admin and compatibility.

Edit:  I just went and double-checked what my settings were, and the three ff7 executables are all default (that is, no special settings or changes from default).  7H has "Run as Admin" enabled, but that's it.  This is within a Win7 Pro x64 virtual machine.

Seems you have two issues going here...
1 - The high DPI issues a lot of other folks have been posting about lately (and, indeed, I see you also posted in one of those)
2 - 55" televisions are 16:9, while FF7 was written in the days of 4:3.

It seems you solved #1 already.  Solving #2 should just require changing the aspect ratio option from "Preserve to fit" to "Stretch to fill" in your Aali driver settings.  You're probably aware that doing so will make everyone look a little... tubby... (not that Wedge wasn't tubby already, but I like my Tifa svelte ;) )

Run it all as admin.  FF7 was released during the days of Windows 98, where everything ran as admin equivalent because Win 9x had no concept of not-admin (NT 3.x and 2k did, but nobody used those for gaming).

Together with how you describe many of the mods causing crashes as soon as on of their functions comes into play, I'm starting to think that perhaps there's something in your install that got corrupted, rather than what you're trying to do.  That, or perhaps Win10's memory protection algos are getting in the way.  Have you tried reinstalling (as admin), and/or running in compatibility mode (Win98 + Admin, or WinXP + Admin) yet?

Unfortunately my host machine doesn't appear to cope very well with using another vcpu, as I get more freezes, it doesn't hang all together though.
You've probably checked this already, but just to be sure, have you enabled VT-x and any other virtualization toggles in your BIOS?

Does virtualbox support 3d acceleration now?
I don't know about Linux hosts, but my setup is a Win10 Pro x64 host, i3-4150 CPU, GTX 960 GPU, 16GB RAM.  The guest I eventually settled on is Win7 Pro x64, 2 vCPU, VboxVGA 256MB, 4GB RAM.  One of the tricks to getting Dx8/9 running is that you have to install the guest additions package while in safe mode.  Whether or not that's required for aali's driver to work I haven't tested, since I never run without the GA package.  Additionally, the new (and eventually better) VboxSVGA graphics adapter doesn't seem to expose OpenGL support as of v6.0.4.

I suspect this is being caused by playing ff7 on a VM. Windows 7 1 cpu, 4gb ram & GPU 1GB, vmware player on ubuntu 18.04:  4 core i5, 16gb ram, Nvidia GTX 1050Ti 4GB
Are you able to feed it another vCPU?  I also play on a Win7 VM, though using Virtualbox instead of VMWare Player, and the only noticeable lag I get with a nearly identical mod list (I turned off Avalanche after leaving Midgar) is when entering into battle.

Looks like it's being initialized successfully... did you perhaps mute or otherwise turn down the volume via the mixer tool in your system tray (as I often do to keep from waking the baby)?

Is that an .IRO you packaged yourself?  I've never seen that mod listed when doing a catalog search/refresh, which makes me wonder if something about it could be the root of your problem.

Fair enough, it's the concept that's important... The idea is to remove DS4Windows as a possible source of your difficulties by using a Windows-native controller.  Whether that's Logitech (I used the F310 only because it's an example of a near-copy of the original PS1 controller), Razer, or whoever doesn't really matter.  It could be that a recent Windows update silently broke a piece of DS4, and simply reinstalling that might solve things.

Don't give up so easily... tell us more about what you've tried (besides just "various tips and tricks"), and what's happening (or not).  Does the controller work for everything except FF7?  Does Cloud move up when you press left on the joystick?  Are the buttons remapped into odd locations each run?  It sounds like you might have a setup that's hard for others to reproduce, but I've been playing games since the days of the Radio Shack TRS-80, and the folks who actually hack (in the true sense of the word!) the game to make these mods have a wealth of knowledge concerning how these things work "under the hood."

100+ views with no replies... so, I decided to have some fun, because what are virtual machines good for if not testing out all sorts of creative... havoc (at one point, Cloud somehow had half a face transplant, while cowgirl Tifa sported a vertex-point waist)  :D  Anyways, here's what's changed since my OP compared to where I am now.  Maybe someone can come up with other stuff to throw at the wall.  It's only a VM (hehehe)

Original: Win7 x64 inside VirtualBox v5.2.22 on Win10 Pro v1803 x64, 7H v1.54
Now: VirtualBox v6.0.2, 7thHeaven v1.55.  Clean install on fresh VM (so stale registry settings wouldn't exist)

Results:  Everything seems to be working except...
Media-Movies: They don't look too different from original to my old eyes, but I did find some giveaways after digging into the IRO that tell me it's not working. An easy one is that I'm seeing the original Eidos logo from:

instead of the cmh175 remastered version (the smoking gun is that cmh's version leads off with a high res "Square Enix" while the original does not)

Text/menu type graphics: These are all still very much original FF7 in style, and nothing like what the library preview images for the "Battle Textures" and "Spell Textures" mods would suggest.  Perhaps I'm missing a mod, or should be running something other than ff7.exe (I have not the Beacause/MO mod)?

I'd try two things...  first, start with a totally clean install using v1.54 and declining the 1.55 update.  Second, instead of using your PS4 or XBox controller with an extra layer of driver abstraction, pick up a physically similar, but Windows-native game pad (e.g. Logitech F310).

Not sure if it's 1-to-1 the same problem I had, but when using a Win98SE virtual machine to play FF7 on a laptop, the keyboard controls would revert back to their installation defaults *ONLY* on character naming screens.  In order to proceed, I had to hold down the Fn button to make the keyboard operate on the keypad-ish secondary meanings, name the characters, and I'd be back to my customized controls.

Yup... PC copy protection at its finest... look for the existence of a specific file in a certain place.  If it's there, you're golden.

(The copy protection built into the PS1 itself, though, was actually pretty creative... there's a neat video on YouTube showing how it worked)

Followed http://forums.qhimm.com/index.php?topic=15520.0... installed using original installer (no Tom's involved) to C:\Games\Final Fantasy VII\, ran the GC without issue, 7H installed just fine.  Got my catalog downloaded/installed and *mostly* activated.  I say mostly because even though I've gone over the load order several times, not everything is actually running.  All of the character (PC and NPC) field and battle models are working, the sound effect changes are working, and the object (potions, materia, etc) replacements are working.  When it was active (I'm out of Midgar, so I've turned these off), the Jessie mod also worked, though the Team Avalanche fields and yarLrson textures did not.  The spellcasting/limit break effects have never worked, and the movie mod does not seem to be replacing anything, either (though I've only triggered two of them so far).  Finally, all the battle/conversation/whatever boxes are still FF7 defaults, not the HD and transparent style I see in so many screen shots.  I've attached my OpenGL config and the APP log from a quick run I just did (inside spoiler tags since I don't see an 'attach' button), which runs between the second-half of Cloud's flashback in Kalm, to exiting the city.
