What's your sound card?  Did you install the yamaha software synthasizer?  (you SHOULDN'T have, cuz it doesnt play nice with xp).

Open your audio properties icon that should be in your toolbar.  make sure "midi" and "wave" aren't muted.

If you dont have a ltitle sound icon, go into start-> control panel -> Sounds and Audio devices.  In the first tab there should be a check box for "Place volume contorl icon in the task bar".    make sure it's checked off.

Short of that, go into ff7config and play around with the different audio settings.

BTW: when you say "the XP patch", you dont mean the Animevamp xp patch, do ya?  Cuz that's a piece of shit which causes more problems than it does any good.  Stick with the official eidos 1.02 and qhimms chocobo racing patches.
