What created these?

Ok, so I've noticed a lot of MODs using this EXACT same setup to apply them but with different text on the fields:



What was used to create them?
I'm really interested as it has a clean visual and it's very small, but I'm not being able to find any clues about it, and since I saw it in so many places already I know it's not custom made, so there must be a little program to set these up like there is for PPFs with PPF-O-Matic 3.0 and stuff.

Any help would be nice. :)
